SPAY/NEUTER VOUCHER APPLICATION 

To be eligible for a voucher, the pet must reside in the Inland Empire and address listed under pet owner's information. Operation Empty Cages's spay/neuter voucher program is intended for those who need help financially to spay or neuter a pet. Once the surgery is approved, you will be issued a spay/neuter approval voucher, at which time we must meet in person to give you the voucher. Scheduling of surgery appointments through approved clinics will be the responsibility of the pet owner. Our spay/neuter approval voucher must be presented at the time of appointment.
